For campuses implementing the 2027 MACHB Survey on your campus, please complete the MACHB Agreement to Participate and complete the informed consent template and return to Kayleigh Greenwood.

If you are a non-PIP campus and wish to participate, please sign this agreement and return the informed consent template.

Timeline:

August 26: 2027 MACHB Agreement provided for signature, and Informed Consent Template provided

September 8: Survey provided for feedback, MACHB Agreement and Informed Consent Template due

September 15: Feedback on survey content/structure due, extra questions due (yes or no)

October 12: Extra questions due (if applicable)

November 16: Implementation dates set

January: Research Team reaches out with sampling guidelines – Samples due two weeks prior to implementation (post-Spring census)

February through early March: Implementation
